One completed form certifying a voter's oath of allegiance to the United States and the state of West Virginia. Signed John Johnson and witnessed by J. Canby, clerk, in the same hand. Johnson swears that he has "never voluntarily borne arms against the United States, the re-organized government of Virginia, or the state of West Virginia." Form was printed for the Township of Martinsburg, Berkley County.
